Total
parenteral nutrition (TPN) and enteral nutrition therapies are prescribed
in cases where the patient cannot receive adequate nutrition by mouth.
With these therapies, nutrition is accomplished via a tube placed
either into the gastrointestinal tract below the area of the disease,
or into the veins.
A registered nurse specializing in nutrition support first consults
with the referring physician, then provides personalized training
for the patient and family members who may actually administer the
treatment. Prior to the start of home nutrition therapy, supplies
and medications are delivered to provide a smooth transition. Horizon

for nursing home staffs. This one-to-one interaction with physician,
home health nurse, patient, and patient family provides continuity
in the plan of care.
